Completing the 2012 – 2013 Illinois Elks Children’s Care Physical / Occupational Therapy Scholarship Application

For the second time, the entire application/instructions and related information for our PT/OT Scholarship Program is only available on the internet.

You may use this information in two ways:

A: You can print out the entire application, complete it and submit
to sponsoring Lodge with all documentation.
OR
B: You can copy the entire application to your computer. Once copied
to your computer, you can complete the application interactively, save it,
print it and submit to sponsoring Lodge with all documentation
