Completing the 2010 – 2011 Illinois Elks Children’s Care Physical / Occupational Therapy Scholarship Application

For the first time, the entire application/instructions and related information for our PT/OT Scholarship Program is only available on the internet.

You may use this information in two ways:

A: You can print out the entire application, complete it and submit
to sponsoring Lodge with all documentation.
OR
B: You can copy the entire application to your computer. Once copied
to your computer, you can complete the application interactively,
print it and submit to sponsoring Lodge with all documentation
